Copy and create course evaluation forms for ETTL and HTW

We won't need this until near the end of the semester. In the Barnicle survey account copy the " CIRTL Network Activity Evaluation' Survey" and make a version for ETTL and HTW. I believe Don and Chris create their own version.  Towards the middle of the semeste remind instructors that they can customize the survey to add specific questions. (see #14)

Add Cafe users name to course spreadsheets

Add a column to the course spreadsheets and list student cafe user IDs so we can keep track of who has created an account.

Once we see if the DCC email is final, we should create versions for the other classes.

 

I don't think we came to a final decision regarding skype. I wonder if we should make it a recommendation as a way to contact Adam if they are having trouble. I think the instructors plan to use skype. Chris and Don were able to use it because there were two instructors. I wouldn't  think Tricia wants to monitor the chat window  in Elluminate and skype.

 Also, since Adam should be reachable by phone, students can always call.
